Mentor Reports

All mentors are required to fill out monthly mentor reports to help the Shapes Mentoring Program staff provide funding agencies with information necessary to continue funding our program. In addition, the staff can stay connected with each match, see progress made, and note any concerns that might need follow-up. This report can be emailed, faxed or mailed to our office.  Please review the instructions below and use the links to assist you with your submission.

Emailing Reports

The “Fillable Mentor Report” can be saved to your computer by “clicking submit by email.”  Please select your type of email and follow the instructions.  If you have a internet email address you may save the “data file” and attach it to an email.  If you are emailing your report, please submit it to stephanie@shapesmentoring.org.  Need help?
